It's month four, the weight is coming down, and you're also more tired than you'd like to be. Maybe a little constipated. Maybe you've noticed more hair than usual on the brush. What the medication is actually doing

GLP-1 receptor agonists, the class that includes semaglutide (Ozempic, Wegovy, Rybelsus) and tirzepatide (Mounjaro, Zepbound), work by mimicking a natural gut hormone called glucagon-like peptide-1. The drugs slow gastric emptying, increase satiety, and reduce the brain's interest in food. They were originally developed for type 2 diabetes and turned out to produce significant weight loss as a side effect, which is now their fastest-growing use.

Three things follow from how they work. First, your stomach takes longer to empty, which is part of why one meal can feel like enough for the day. Second, your hunger and thirst signals both quiet down. The same receptors that handle appetite seem to also dampen the urge to drink, which is why dehydration is a recognized risk on these medications independent of food intake. Third, because you're eating less and absorbing nutrients differently, the supply of protein, fiber, and micronutrients reaching your tissues drops, sometimes substantially.

A cross-sectional study of GLP-1 users published in 2025 found caloric reductions ranging from 16% to 39%, with the majority of users falling short on calcium, iron, vitamin D, and B12. A retrospective observational study of adults with type 2 diabetes on GLP-1 therapy documented vitamin D deficiency in 7.5% of users at six months and 13.6% at twelve months. These are not edge cases. They are predictable consequences of eating less without a plan for what the smaller plate has to contain.

Pillar one: protein and muscle

This is the most consequential of the four pillars and the one most likely to surprise people. When you lose weight on a GLP-1, a meaningful share of what you lose is lean tissue, including skeletal muscle.

The numbers are in the major trials. In the body composition substudy of STEP 1, the trial that earned semaglutide its weight management indication, lean mass dropped 6.92 kg, or 13.2% from baseline, and accounted for about 39% of the total weight lost. The SURMOUNT-1 substudy of tirzepatide, the trial behind Zepbound, found that lean mass loss was about 25% of total weight lost. A 2024 review pooling recent GLP-1 trials estimated lean soft tissue loss at 26% to 40% of total weight lost depending on the medication and dose.

Some loss of lean mass is expected with any significant weight loss. The concern with GLP-1 medications is that the absolute caloric deficit is often larger and the appetite suppression makes it harder to hit a protein target. Protein requirements during weight loss are higher, not lower, because protein is what tells your muscles to hold on. The current evidence supports a daily intake of roughly 1.2 to 1.6 grams per kilogram of body weight for adults losing weight, and athletes or people in steeper deficits may need 1.6 to 2.4 grams per kilogram. For a 180-pound adult, that's roughly 100 to 130 grams a day.

Protein alone is not enough. The 2025 joint advisory from the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association, and the Obesity Society was explicit on this point: increased protein intake without resistance training is unlikely to preserve muscle mass during GLP-1 therapy. One review of GLP-1 users on a structured exercise protocol found that those who did resistance training preserved roughly 60% more lean mass than those who didn't.

Two more details matter for muscle. First, protein is more useful when it's distributed across the day rather than dumped into a single meal. Each meal containing roughly 25 to 40 grams of protein appears to maximally stimulate muscle protein synthesis, with the response governed in large part by leucine, a branched-chain amino acid abundant in whey and dairy. Older adults, and likely anyone with reduced food intake, need slightly more protein per meal to clear the same anabolic threshold. Three to four protein-anchored meals a day will outperform one big steak.

Second, the resistance training itself doesn't need to be elaborate. Two sessions per week of compound movements (squats, hinges, presses, rows, carries) at a real but submaximal intensity will protect most of the muscle you have. The threshold isn't athleticism. It's giving your skeleton a reason to keep its scaffolding. Walking, while excellent for cardiovascular health and digestion, doesn't substitute for resistance training in terms of muscle preservation.

The practical version: aim for protein at every meal, weigh yourself once a week and your strength once a week (how many push-ups, how heavy a carry), and add resistance training two or three times a week. Pillar two: fiber, digestion, and the gut

GLP-1 medications slow gastric emptying as a feature, not a bug. That's part of how they make you feel full. The same mechanism is why constipation is one of the most common side effects.

In the STEP 1 trial of semaglutide, 23.4% of patients reported constipation, compared to 9.5% on placebo. Nausea hit 44.2%, vomiting 24.8%, and diarrhea 31.5%. Most events were mild to moderate and improved over time, but for a sizable group they don't fully resolve, and they can flare again with each dose increase. Side-effect timelines vary, but nausea and GI complaints typically peak in weeks one through three of starting or escalating a dose and improve from there.

Two things drive the constipation. The first is the slowed transit itself. The second is that you're eating less food and therefore less fiber, less fluid, and less of the fermentable substrate your gut bacteria need to do their work. Most adults already fall short of the recommended 25 to 38 grams of fiber per day. On a GLP-1, with appetite cut by a third, that gap widens.

More fiber alone isn't enough. The right kind of fiber paired with adequate fluid is what moves things. Soluble fibers like psyllium, inulin, and partially hydrolyzed guar gum draw water into the stool and feed the bacteria that produce short-chain fatty acids, which signal the colon to keep moving. Probiotic strains, particularly Bifidobacterium lactis, have shown modest but consistent benefit for stool frequency and consistency in adult constipation, including in a 2024 meta-analysis pooling randomized controlled trials.

There's a separate, intriguing line of research on a specific gut bacterium called Akkermansia muciniphila, which produces a protein called P9 that stimulates the body's own GLP-1 secretion. That doesn't mean drinking a probiotic shake replaces a GLP-1 prescription. It does suggest that the gut bacteria you feed have a meaningful relationship with how the medication works, and that supporting them with prebiotic fiber is more than a comfort measure.

Pillar three: hydration and electrolytes

Hydration is the most underrated of the four pillars because most people assume thirst is a reliable signal. On a GLP-1, it isn't.

GLP-1 receptor agonists suppress water intake through a mechanism that appears to be at least partly independent of their effect on appetite. Add to that the smaller meals (which themselves carry less water than people realize), the occasional bouts of nausea or vomiting that can come with dose increases, and the slowed gastric emptying that makes drinking large volumes of fluid uncomfortable, and you have the recipe for a chronically under-hydrated person who doesn't feel thirsty.

Mild dehydration on a GLP-1 looks like fatigue, dizziness on standing, headaches, dark urine, and the kind of muscle cramps people often misattribute to "the medication" generally. Severe dehydration can land someone in the ER. The Cleveland Clinic and the American College of Lifestyle Medicine both flag adequate hydration as one of the cornerstone behaviors for anyone on the medication.

Plain water is fine and necessary, but it's not always sufficient. When food intake drops, sodium intake drops with it, since most dietary sodium comes from prepared foods. Potassium and magnesium, which help muscles contract and relax properly, also slip when fruit, vegetables, and whole grains thin out of the diet. Many people end up technically euhydrated by total fluid volume but functionally short on the electrolytes that make hydration work.

Pillar four: GLP-1 vitamins and minerals

The fourth pillar is the quiet one. GLP-1 vitamins and minerals don't produce dramatic symptoms in the first few months, which is why people tend to ignore them.

The vitamins to take with GLP-1 medications, based on the deficiencies most commonly documented in users, are vitamin D, iron, vitamin B12, calcium, and to a lesser extent thiamine and folate. The Frontiers cross-sectional study found more than 60% of GLP-1 users consuming less than the estimated requirement for both calcium and iron. Harvard Health, citing similar research, has flagged this risk publicly. The patterns echo what's been seen for years in bariatric surgery patients, who eat less for similar reasons and need formal nutritional support to avoid deficiencies, a parallel several recent reviews have drawn directly.

A few specific notes worth knowing. Vitamin B12 absorption depends on stomach acid and intrinsic factor, both of which can be affected by the slowed digestion the medications produce. Iron is harder to come by when you're eating less red meat and fewer fortified grains. Vitamin D is the most common single deficiency, and it's worth getting a baseline level checked through your prescriber. Calcium and vitamin D together matter more than usual on a GLP-1 because rapid weight loss is associated with bone density changes, and the literature on bariatric patients suggests this is where the long-term risk shows up if it's not addressed early.

These are not arguments to take ten supplements. They are arguments to know what your diet is missing and to either fix it through food or fill it intentionally. Timing matters more than people think

If you're on a weekly injectable (Ozempic, Wegovy, Mounjaro, Zepbound), most people experience a predictable rhythm. The first 24 to 72 hours after a dose tend to bring the strongest appetite suppression and the highest probability of nausea. By day four or five, hunger usually returns enough to make protein-focused eating easier. By the end of the week, most people are eating closer to a normal pattern, then the cycle resets.

Use that pattern instead of fighting it. On the first day or two after an injection, prioritize liquids and easily digestible protein: a protein shake, broth-based soup, Greek yogurt, scrambled eggs. Save your harder-to-tolerate foods (high-fat meals, raw vegetables, large portions) for the back half of the week when your stomach feels closer to normal. Spread protein across smaller meals rather than trying to compress it into two large ones, since smaller meals are easier on a slowed gut.

If you're on oral semaglutide (Rybelsus), the rhythm looks different because the dose is daily rather than weekly, and the drug exposure stays steadier. Side effects tend to cluster in the first few weeks of starting and after each dose escalation rather than peaking and ebbing within each week. The other constraint with the pill is the morning dosing window: you take it on an empty stomach with no more than four ounces of plain water, then wait at least 30 minutes before eating, drinking, or taking other medications, so the absorption isn't disrupted. That window is also the right time to start hydrating with plain water; save your electrolyte drink and protein shake for after the 30-minute mark.

Dose-escalation periods are their own category, regardless of which formulation you take. Each step up tends to bring back a milder version of week-one side effects for several days, sometimes as long as two weeks. Plan your titration weeks the same way you'd plan your first week: liquids and protein-forward meals up front, easier appetite later. If a dose increase produces side effects you can't manage with food adjustments, that's a conversation with your prescriber, not a personal failing.

A worked protein example

A 180-pound adult aiming for the lower end of the protein recommendation needs about 100 grams per day. On a typical reduced-appetite day, that might look like:

A morning protein shake with 25 grams of whey or whey-and-collagen blend. Two eggs and a slice of cheese around mid-morning, roughly 20 grams. A four-ounce piece of chicken or fish at the largest meal of the day, around 30 grams. Greek yogurt with a handful of berries in the late afternoon, 15 grams. A small evening snack of cottage cheese or a protein bar, another 10 to 15 grams.

That's six small protein hits across the day rather than two large ones. It's also why a protein supplement matters here. A 25-gram morning shake covers a quarter of the daily target before you've fully woken up, which is the easiest possible way to bank protein when your appetite is unreliable. Lower-calorie protein options matter on a GLP-1: you're working inside a tighter caloric envelope than you're used to, so a protein source that delivers 10 to 25 grams without bringing 200 calories of sugar with it is much easier to fit in.

GLP-1 side effects and nutrition: what your symptoms are telling you

Most of the discomfort people describe in months three through nine maps to one of the four pillars. GLP-1 side effects and nutrition are tightly linked, but the translation from a specific symptom back to a specific gap isn't always obvious in the moment. A few common patterns worth knowing:

Persistent fatigue and afternoon crashes often trace back to either dehydration or a low protein day, sometimes both. Energy without enough protein looks like blood sugar swings; energy without enough water looks like a slow-building heaviness that coffee briefly papers over. If you've ruled out short sleep, run the protein and water audit before assuming the medication itself is to blame.

Cramping in the calves, eyelid twitches, or restless legs typically point to electrolytes, particularly magnesium and potassium. Both drop when produce and whole grains thin out of the diet.

Constipation that responds to neither prunes nor coffee usually needs a soluble fiber paired with extra fluid, not a stimulant laxative. Stimulant laxatives can work in a pinch but become less effective with regular use, and they don't address the underlying mechanism.

Hair shedding that starts two to four months after a significant weight reduction is often telogen effluvium, a stress response in the hair follicle to rapid weight loss and the nutritional shifts that come with it. The literature suggests it's largely reversible with adequate protein, iron, and zinc, and it tends to resolve on its own as the weight curve flattens. The systematic review on GLP-1-related alopecia found that 5.3% of Wegovy patients with greater than 20% body weight reduction reported hair loss, compared to 2.5% of those losing less than 20%.

Brain fog, brittle nails, mouth corner cracks, or numbness in the hands and feet are all worth flagging to your prescriber. These can indicate B-vitamin or iron deficiencies that warrant lab work rather than guessing.

Labs worth asking about

A blood panel is the difference between informed action and the supplement aisle's best guess. The labs most worth asking your prescriber to run, especially in the first six to twelve months, include:

  • 25-hydroxyvitamin D, since vitamin D deficiency is the single most common deficiency documented in GLP-1 users
  • Ferritin and a full iron panel, given the documented gap in iron intake among GLP-1 users
  • Vitamin B12 and folate, since both can be affected by reduced animal protein intake and altered gastric absorption
  • A basic metabolic panel including sodium, potassium, and magnesium if you're having any cramping, dizziness, or palpitations
  • Thyroid function and a comprehensive metabolic panel if fatigue persists

Your prescriber may already be running some of these. If you've never had a baseline drawn since starting the medication, it's a reasonable thing to ask for at your next visit.

How to actually use this

The four pillars are not a checklist to grind through. They are a framework for noticing where you're slipping and acting before it costs you.

A practical weekly habit: at the end of the week, ask yourself four questions. Did I average roughly 0.7 to 1.0 gram of protein per pound of body weight per day? Did I have a bowel movement most days, and if not, what was different? Did my urine stay light yellow or did I see dark concentrated mornings? Did I eat at least one serving each of leafy greens, a colored vegetable, and an iron-bearing food? If you can answer yes to all four, you're probably doing fine. If you're consistently missing one, that's where to focus next week.

Resistance training, twice a week minimum, is non-negotiable if you want to keep the muscle you have. It doesn't have to be a barbell program. Bodyweight squats and pushups, resistance bands, or a beginner gym routine will all work. The point is to give your muscles a reason to stay.

Sleep, stress, and movement matter too, but they sit on top of the four pillars. If protein is short and you're dehydrated, no amount of meditation will close the gap.

The mistakes that come up most often

A few patterns show up repeatedly in the people who feel worst on a GLP-1, and most of them are correctable.

The first is drinking water without electrolytes. You can drink three liters of plain water a day and still feel cramping, dizziness, and fatigue if your sodium and potassium are running low. Hydration is volume plus electrolytes, not volume alone.

The second is going low-carb because someone online recommended it. Reduced appetite already keeps blood sugar steady, and aggressive carb restriction layered on top of that often produces lethargy and brain fog without delivering additional weight loss. The medication does most of the heavy lifting on appetite. You don't need to add a second restriction strategy.

The third is letting cardio crowd out resistance training. Walking and steady-state cardio feel easier when your appetite is small, but they don't preserve muscle the way lifting does. The literature is unambiguous on this point.

The fourth is treating supplementation as a swap for food rather than a complement to it. A protein shake is helpful when it's hard to eat enough, not a substitute for the chicken and vegetables you'd otherwise have at dinner. The same logic applies to fiber and electrolyte powders.

The fifth is not asking for labs. Most prescribers will run them if you ask. Most won't volunteer them in a routine GLP-1 follow-up. A baseline at six months is worth the appointment.
